How do I find a participating pharmacy?

How do I find a listing of covered prescription drugs?

View the appropriate CDPHP formulary by logging into your CDPHP Account.

How do I fill my prescription at a pharmacy?

Prescriptions must be filled at a CVS Caremark participating pharmacy. Show your ID card when filling a prescription to be charged the applicable co-payments for your prescription drug program.


How can I submit a claim for a prescription paid for out-of-pocket?

Is Mail Order Service available?

Yes. CDPHP members who have prescription drug coverage may be able to order maintenance drugs, or drugs taken on a regular basis from CVS Caremark Mail Service Pharmacy, CDPHP's mail service pharmacy vendor. Up to a 90-day supply of medication can be ordered through this program. Once a member's prescription is on file with CVS Caremark Mail Service Pharmacy, members can order refills by mail, phone or online and have their medication delivered to their door.
